Policies - No Show

NO SHOW POLICY & PROCEDURE FOR OFFICE VISITS AND PROCEDURES (01/14/16)

1. Cancellation/No show policy for office visits.
We understand that there are times when you must miss an appointment due to emergencies or obligations for work or family. However, when you do not call to cancel an appointment, you may be preventing another patient from getting much needed treatment .

** For established patients, if an appointment is not cancelled at least 24 hours in advance, you may be charged a twenty five dollar established patient fee. This fee will not be covered by your insurance and will need to be paid in full prior to being rescheduled for another appointment.

** For new patients, if an appointment is not canceled at least 24 hours in advance, you may be charged a fifty dollar new patient fee. This fee will not be covered by your insurance and will need to be paid in full prior to being rescheduled for another appointment.

** If a patient has two No show appointments, chronic last minute cancellations, or chronic lateness, they may be discharged from our practice. At that time, our providers will only be available for emergent care for thirty days and a new medical provider will need to be established by the patient.

2. Cancellation/No show policy for outpatient procedures.
Due to the large block of time needed for your procedure, last minute cancellations can cause problems and added expenses to the office. If procedures are not cancelled at least three days in advance, you will be charged a seventy five dollar fee; this is not covered by your insurance and will need to be paid prior to being rescheduled.
